COACH supports activities to improve trust and communication, bridge patients to health-related needs and while exploring ways to increase access to social needs.
Through the collective feedback provided by partners, COACH recently created a series of five video-based modules that can be integrated into the learning management systems of all COACH participating health systems in the region. Learners can watch each module in the suggested order, or can assign certain modules to team members depending on their role in assessing and addressing social needs.
